Thank you for your interest in the Dell Children’s Medical Center Teen Summer Volunteer Program!
We will be accepting applications for Summer 2011 from March 21, 2011 through April 1, 2011.
In order to become a Teen Volunteer you must be 15 years old as of June 1, 2011. Sorry, this is a hospital policy and we are unable to make any exceptions. In addition to this you must also be willing to fully commit to our 8 week program. We will allow for one week vacation time over the summer, but we must be notified two weeks prior to the vacation date so we can notify your service area of your absence.
Our summer Teen Volunteers will either work on:
Monday, Tuesday, Wednesday or Thursday - 9:00am-12:00pm or 1:00 pm-4:00 pm
The program will run from June 6, 2011 until July 28, 2011. The total cost of the program is $15 for a white polo shirt and $5 for ID badge (to be paid separately to each office). These fees will only apply to those who are accepted in to the program, so please do not send any money at this time.
To assure that volunteers are free from communicable disease, all volunteers will be required to have a current TB skin test before beginning their assignment. In order for the TB test to be valid, it must have been administered no earlier than August 7, 2010. Volunteers are responsible for seeking their individual medical providers regarding all immunizations and TB tests.
In addition, all persons born after 1957 will be required to show proof of the following immunizations or immunity: rubella (3 day or German Measles), 2 live vaccinations against measles (rubella) since 1968, and chicken pox (either dates contracted or date of immunization).
Remember to include a copy of your immunization record (measles, mumps, rubella), a current transcript, one letter of recommendation, have your parent's signature on the application, and sign the teen volunteer pledge form. We will not accept incomplete applications.
Unfortunately, spaces in the summer Teen Volunteer Program are limited. To insure that you application will be considered, please send a complete application.
